Form 13H is a black and white sealed spreading rate chart. The top and bottom are a stripped pattern, the top part of the middle section black and bottom is white. Overall dimensions of 11-1/4 x 17-1/4 in (286 x 438 mm). The design area of the chart covers 1000 cm squared, to help with calculating coverage.
